I've got it now. A slight correction to my previous post....
Did this occur in a hotel that has since been torn down located in a more western state than New Jersey?
Like maybe the Leavenworth Planter's Hotel in Leavenworth Kansas?

However, accoding to the Governments new Unafordable Care Act, there is the possibility that a loop hole exist that might provide him coverage. (no one really knows yet)

Unfortunately Booth was dead in slightly over two years following the surgery, and so the government would not have processed his paperwork yet, and therefore would have never paid the claim.
So when is this "Old Enough To Know Better" supposed to kick in?
