Abraham Lincoln once gave a speech from the steps of this hotel. John Wilkes Booth stayed here while performing in the town it was located. Where was this hotel and what was its name?

Like maybe the Leavenworth Planter's Hotel in Leavenworth Kansas?

Lincoln was there in 1859, and Booth stayed in the hotel in late 1863 (p. 139 of Art Loux's book).

A strange incident happened on October 12, 1860, in Columbus, Georgia, at the Cook's Hotel. Matthew Canning accidentally shot John Wilkes Booth in the thigh (or thereabouts) as the two men were "playing" with a pistol in Booth's room.

Feeling bad, Canning scheduled a benefit performance (Julius Caesar) for John Wilkes Booth.

This is the amount Booth received from the benefit performance?

On Monday, April 13, 1863, Dr. May removed a fibroid tumor from Booth's neck. This operation took place in Washington.

Is this the amount Booth's health insurance company reimbursed him for Dr. May's surgery?

Being in the insurance industry, I can answer that for you Roger.

However, accoding to the Governments new Unafordable Care Act, there is the possibility that a loop hole exist that might provide him coverage. (no one really knows yet)

Unfortunately Booth was dead in slightly over two years following the surgery, and so the government would not have processed his paperwork yet, and therefore would have never paid the claim.

Booth submitted a claim for payment for his service in the Richmond Grays. He received a payment of $64.58.
