Naturally occurring malaria is very rare in the U.S. now since we pretty much eradicated it in the 20th century. Most cases here come via people traveling to areas on the globe where it is still prevalent. I know that it was an ongoing threat in the American South up until the concentrated efforts to eradicate it. The biological end of it is more than I can discuss.
